Our intake process is the first step toward connecting veterans with the support they’ve earned. We take the time to listen, understand, and assess each individual’s needs—ensuring they are linked with the right resources from the very beginning. Through personalized interviews and assessments, we create tailored support plans that address challenges such as trauma, housing, or legal issues. With open, compassionate communication, CVJP makes the transition into services smooth and empowering, helping veterans take confident steps forward.

Veterans with pending criminal cases should complete our Statute Intake Form

Veterans with non-criminal concerns should complete our General Intake Form
Suicide Risk Assessment Survey:

Complete a Suicide Risk Assessment Survey (required before submitting any intake form)

How to Refer a Veteran

What Happens Next
Once the referral is received, a CVJP team member will reach out directly to the veteran. We’ll guide them through the intake process, connect them with tailored support, and walk with them every step of the way toward healing and resolution.
